The wound after breast surgery should generally be checked 24-48 hours later. The gauze dressing should be removed, the steri-strips left on and the patient can shower. Dry the steri-strips with a hair dryer on cool setting. Replace gauze if necessary and hold in place with a bra.

I would recommend using existing incision site through which the infra mammary fold can be placated to the chest wall with or without acellular dermal matrix reinforcement. Going smaller would also help increase your chances of a better result. I think the two breasts are relatively similar and should be treated in the same manner.

You have bottoming out of the implants and need to have the infra mammary fold established to its original position with suturing of the fold to the chest wall with or without the use of acellular dermal matrix. I would not go larger but reuse same implants or go smaller.
