There are many variables such as saline or silicone gel implants and type of lift needed. I usually use silicone gel implants which cost about $900 more than saline and most commonly use a circumvertical lift technique. I would be impossible for anyone to tell from your picture what cc implant you would need. I like to look at pictures with patients and discuss what look you are wanting and then I use a temporary sizer in the operation room to see what it takes to deliver that. After doing over 3500 augmentations I have found that this works very well. From your photos I would think that you would need a breast lift along with the augmentation to have a nice result. You have enough fatty tissue in the abdomen for at least a moderate buttock augmentation. From the appearance of your breasts now and from what you are wanting I would think you will have a very nice result with either type of implant. There are many things that you need to consider and nothing can take the place of an actual consultation. Make sure you see a board certified plastic surgeon and bring in photos of what you are wanting for an end result. Unfortunately it is impossible for you to decides the amount of cc’s, profile of implant and other decisions…

By using high profile implants you should be able to go bigger without a problem. I know many people advocate trying on sizers but I have found that just sticking an implant on top of your breast does not really predict what you will look like after surgery. I use temporary sizers in the actual surgical pocket in the operating room which give a perfect predication of what size you need. Most of my patients can resume a desk job in about 3 days, most other work in about a week but no lifting over 40 pounds for 6 weeks. Recovery varies some for every patient however. I do the majority of my augmentation mastopexy procedures as one combined surgery. The fact that you want to use a large implant and have a lot of sagging I would recommend having a lift first and then an augmentation as a second surgery. Of course it is possible to get a DD bra size with augmentation without a lift. But if you do have a significant amount of sagging you will probably need a lift. These can be difficult decisions so make sure you are seeing experienced Board Certified Plastic Surgeons.Sincerely,Gary Hall, MD

From looking at your pictures I believe that if you chose to have surgery you have no choice but to have a lift. If you were my patient I would tell you that you would be better off not having any surgery if you were not going to have a lift. My breast augmentation op is next week and i’m worried than the implant size (325cc) will be too big for

In my practice I use a temporary sizer in the operating room to decide on the exact implant but would estimate a range in the office. It is impossible to tell from just looking at photos what size would be best for you but I would not think 325 would be too big. I used to use tear drop implants 18 years ago but quit using them as I think they usually give a less natural result, at least with saline. The new form stable shaped implant seems to do a better job however…
