Hello. You have two questions.
First, I teach my patients there are different "fill" volumes. It really depends on the RANGE of your implants. Most 270cc will fill only to 300. Check on this. There is minimal fill volume and optimal fill volume (the range of the implant) then overfill. The latter almost always leads to rippling so i never overfill beyond the maximum (optimum) fill. Some implants with a broad range of fill volume i fill to the mid range.

From your photos, my sense is you would obtain a very natural and beautiful result with the correct positioning of your implants. This is a very often performed surgery by plastic surgeons; remove the implant and the capsule, especially the lower portion, release the scar tissue at the bottom of the breast, allow the implant to go to the lower pole BEHIND the nipple areolar complex where it belongs and use a slightly bigger implant. I have done calf implants on many men and women with just exactly what you have. Implants do not make spider veins better or worse. You are at no increased risk. It may be good to evaluate your veins system, but mostly only if you have varicose veins. I have had only very happy patients after calf augmentations.

Be careful. As a doctor that sees a lot of patients that have had facelifts before, one common problem is a pucker or persistent fold at the end of an incision line. This comes from making the incision too short and leaving a fold, often a large one. Always plan and discuss the incisions with your doctor. A short incision is fine if you don't have too much looseness. A longer incision is needed to accommodate a lot of looseness, and thus, provide a better result. The plan should be matching...

For your photo a facelift is better. Think facelift for loose hanging skin and folds. Think laser for the TEXTURE of your skin. It improves texture, color, pigmentation very reliably. If you get tightening from a laser or nonsurgical procedure it is always minimal...and a bonus.

Fillers are to add volume and fill out furrows and wrinkles, and if it works, provide some lift as a bonus. They are not meant to replace a facelift. Facelift is for loose hanging skin, loose ligaments and muscles, and extra fat that needs contouring. It looks like from your photos facelift is best, maybe with some filler or fat graft in your laugh lines.

There is a lot of work through a small incision in these cases. sometimes the incision edges heal slowly due to all the stretch during surgery. If the incision gaps open a tiny bit when sutures are removed like yours, I apply stern-strips right away. This prevents a gap and allows the incision to heal quicker. Check with your PS.

Acellular dermal matrix in regards to capsular contracture

YES on STRATTICE

Strattice is the only acellular dermal matrix approved for capsular contracture. It works so well at preventing recurrence that I refuse surgery on patients that have contracture and do not agree to placement of this product. It is quite technique and management sensitive so make sure your PS has experience with this.

Sutures in the capsule and the possibility of some dents or flat areas virtually always round out because they assume the round shape of the implant over time. Once the internal surgery has healed, i have my patients do massage. Ask your PS.

Most of the time your breasts look better; slope to the top, rounder bottom, nipple in middle. You may have a few minor folds.It really depends on how much breast tissue you have as it relates to implant size -- the more breast tissue, the less changes. Good idea to do this prior to explant and leave the deflated implants in for a while.

Will explant alone likely result in a double bubble look because my crease was lowered in original BA?

If anything, you will look much better and the IM crease will go back where it came from. Once the implant is removed, have your PS remove the capsule and the area below the crease will go back to your abdomen-- where it came from. Sew it down if needed. The crease will return to normal and your breast tissue should resume its normal shape. It may have creases for some time so massage these. Rarely do i need to do a lift, but if in doubt, it can be safely done later.

I think your results, even after breast feeding, are very nice. But I understand that you see what all women see either after breast feeding, or just over time, or even right away after BA: lack of some upper pole fullness. This may be due to just the low "foundation" of your breasts on your chest wall, or a long chest, or some relaxation of the breast implant "envelope" as you have had. I think just an internal capsular repair to lift the implants to better behind...

I do not operate on patients currently smoking or vaping. Electronic cigarettes still contain chemicals that cause constriction of your blood vessels. This will ALWAYS lend SOME degree of risk to your procedure in terms of healing and scars. So why should you or I take this risk?

Yes this can be cured. It is challenging but possible. It depends on the size and location of your implants and your anatomy. Often exchanging location such as above to below muscle, using smaller implant, and suturing your CAPSULE tissue together medially AND reinforcing with Strattice. Post op garment that compresses midline is helpful.

Does a breast implant revision hurt as bad as the first time?

NO!

In my hands and my experience I have never had revision hurt as much as the primary surgery. The answer is simple: you already have the incision, you already have the space, you already have the muscle stretched out. Add to this a FIELD BLOCK of novocaine around the edges during surgery and you can expect a much more comfortable surgery. It is more technical, buy much less painful and a quicker recovery regardless of the extent.
