Why Can’t my Surgeon Choose my Implants?
Who chooses breast implant size?
Jul 10, 2012
Both the patient and the surgeon should be involved in the implant decision. The surgeon should have an understanding of the “look” or “size” that the patient desires. Conversely, the patient should have a goal of how she wishes to look in clothing, swimwear, and even nude. Asking for a cup size is not a reasonable goal. Sometimes photos are helpful for the surgeon, because of the different perceptions by the patient and the surgeon conceding cup size. I have…
Should Fees for Anesthesia and Operating Room Increase for Adding an Additional Procedure Being P
Operating Room fees
Jul 10, 2012
Operating room fees are usually charged by time, as are anesthesia fees. Combining procedures is actually more cost efficient because most operating room and anesthesiologist fees begin the moment you enter the operating room. Assuming there is a 15 minute lag until you are put to sleep, and also after you wake up, you will save money by not having two operations and replicate the O.R. “inflow-outflow” time period.
Risk of Nerve Damage with Chin/neck Liposcution?
Neck Liposuction and nerve injury
Jul 1, 2012
Though liposuction uses blunt cannulas, there is a small, but appreciable chance that a nerve, most commonly the marginal mandibular nerve to the corners of the lip, can be traumatized. When this occurs, the affected side will not be able to “frown” and smiling will occur only on the opposite side. This injury is usually temporary and will resolve in several weeks time.
Closed or Open Rhinoplasty – What Do I Need Done?
Open vs. closed rhinoplasty
Jun 21, 2012
An experienced board certified surgeon should be highest on your list for a rhinoplasty. Your nose can be changed by either method. However, reducing your hump only will leave your nose too wide and your tip too large. Balance of all the nasal structures is more important than the method used.
Do I Need a Lift? I’m 50 y/o and Don’t Want To Look Like I’m 20, but Want Them Larger?
Lift or No Lift
Apr 30, 2012
If you put a pencil under your breast at the inframammary fold (lower crease) and your nipple is below the level of the pencil, you most probably would benefit from some sort of lift along with your augmentation. Otherwise, you will be left with the "Snoopy" look: lax and loose breast gland hanging down from the curvature of the implant.

It may be that you only need to lift the nipple/areola (peri-areolar lift), or possibly a vertical lift to map them…

What’s the Disadvantage of Open Rhinoplasty?
Open rhinoplasty
Mar 6, 2012
In experienced hands, both open and closed rhinoplasty give a surgeon options to achieve the desired outcome. An open rhinoplasty will leave a scar in the columella, which is usually imperceptible when healed. Also, an open rhinoplasty may have a slightly longer time of tip swelling.
Does Health Insurance Cover Lipo?
Insurance and liposuction
Mar 6, 2012
Liposuction is a cosmetic procedure. No insurance company offers payment or coverage for cosmetic surgery.
Vaser Lipo or Tummy Tuck or Other?
Tummy tuck vs. liposuction
Feb 24, 2012
The question that needs to be asked to determine what options are right for your is, "If the fat around my trunk is removed, will my skin shrink-wrap enough to look smooth, or will it stay loose and create folds?"

In general, abdominal skin with stretch marks does not have the elasticity to shrink or contract. Smoother skin with taut fat (not loose, shaky fat) may tighten adequately. I find that I do liposuction in almost all my tummy tuck patients.

Can I Get a Nose Job for Free if I Have Been Referred to by a Doctor for my Nose?
Cosmetic and reconstructive nasal surgery
Feb 10, 2012
Your insurance will pay a portion of your nasal surgery for the reconstruction from trauma. You should understand that they will not pay for all of this portion, and you should check with your insurance documents regarding your deductible and co-pay. The cosmetic portion, to "shave down parts….I dislike" will not be covered. You will pay for this portion along with the deductible and co-pay out of your pocket.
Can I Elevate my Tip to Disguise my Hump Without Dorsal Rasping?
Options in rhinoplasty
Feb 10, 2012
From your photos, your tip will require some sort of support and/or projection, whether it be a tip graft to augment the tip and/or a columellar strut to support the tip. Your hump will not be "disguised" by these manuevers and may still requires some reduction.
What Procedure Accomplishes This Kind of Upward Diagonal Correction?
Fullness around the mouth and facelift
Feb 8, 2012
In general, the farther away a deformity is from the incision, the less effect any surgery will have on the problem. Your specific issue is in an area rarely directly lifted or tightened by any kind of facelift. The pull of the skin in any surgery progressively diminishes as the distance to the problem grows, despite what you see when you pinch your skin. You will be disappointed with the results in an extended surgery, and there is a high possibility of injury to the…
Would a Arasys Fix my Lose Skin the Dr Left Me After a Tummy Tuck?
Tummy tuck poor result
Feb 4, 2012
I am in agreement with Dr. DeMars; unless you had severe abdominal scarring from previous procedures such as a hysterectomy or other major abdominal procedures, your result appears not to have the hallmarks of a procedure performed by a board certified plastic surgeon, especially with regards to the position of the scar, the management of the navel, and the result of the upper abdominal skin.

You should seek a second opinion with plastic surgeon certified by the American Board of Plastic…

I Am Trying to Decide Between 304 and 339cc Silicone Subpectoral. Which Size Would the 339cc Pla
What size of implant…
Jan 16, 2012
Implant volume and cup size are not equal in women: meaning a 5'4" patient versus a 5'9" would have different cup sizes with the same implant. A surgeon usually asks, and tries to produce, a certain "look" irregardless of final cup size. Most bra manufactures do not standardize their cup sizes.

The 35cc difference that you are concerned about is a little over 2 tablespoons; not really much volume difference. The implants differ in…

Compound Problems After Breast Augmentation. Advise Please.
Breast implant revision
Jan 14, 2012
I usually suggest waiting 3 to 4 months prior to any revision to allow your body to soften and mature the wounds. From your photos you may require a repair (plication) of the right lower pole with a possible release of the transverse band and/or scoring of the deep underside of the breast gland. Concomitantly, a release (capsulotomy) of the left inferior pole may also be needed.
I Have Excess Fat in my Thighs, Stomach, and Back. What Are My Options?
Liposuction for weight loss
Jan 3, 2012
Liposuction is not indicated for weight loss. Your BMI (body mass index) is 38.3; obesity is defined as a BMI greater than 30. Your risks of any surgery are also increased. You should lose weight to less than 180 lbs. (BMI of 30) for a safer and better result.
