This is not an easy question to give an exact answer because there are multiple structures that will need to be reconstructed. If you are able to get evaluated at one of the centers doing facial allotransplantation (NYU, Harvard, Cleveland Clinic, etc.) that would be your best bet if you are a candidate. It’s difficult to tell exactly what’s going on from photos since the post-op is slightly at an angle. In your pre-op photo it looks like there is a small size discrepancy with a slightly outward point to the nipple on the right. It appears that the right eyebrow is sitting a little lower in this picture which is causing the skin of the upper eyelid the bunch and obscure the upper eyelid crease on that side. The best thing you can do at this point is avoid picking at the area and try to keep it clean. Often I recommend an antibiotic ointment to keep the area moist. Open wounds will tend to contract and pull the eyelid down causing scleral show. The blood flow to that portion of the eyellid may be poor due to anatomy, injury, or level of dissection.

Most of the time fat necrosis will present as a firm nodular area. Fat grafting can often be extremely unpredictable. Its important to let your surgeon know that something is different. He or she may know the specifics of your case that may be at play.
