Facial volume loss with hollowing
May 19, 2016
Congratulations on your weight loss!  One of the changes we see with weight loss is the loss of facial  volume which you are seeing now in the cheek and under eye region.  You may well be able to manage this volume change with a filler, understanding that many fillers last only 1-2 years, or with fat grafting.  You will need to be seen in person to assess the changes and to have a good discussion of the different options for treatment.
How do I fix my under eye hollows / bags?
Tear trough
May 19, 2016
Thank you for including your photos.  I notice that you have significantly asymmetric lower eyelids with the left side trough deeper and with more bulging of your fatty tissues than the right side.  While augmenting the tear trough with filler can help soften the appearance of the groove, I suspect that the amount of fat bulging that you have on the left side will prevent you from getting the kind of improvement you are seeking.  Re-positioning of the bulging fatty tissue into the tear tr…

Lift versus fat grafting?
Aug 24, 2015
You appear to have lost some of your breast volume after breast feeding/pregnancy, a condition called post-partum atrophy. In general, there are two options for restoring volume to breasts and improving sagging: augmentation with implants or augmentation with fat grafting. With augmentation one can choose to add the preferred volume and it would definitely give fullness to the upper breast as well as increasing projection of the breasts. Augmentation can be done and…
What is the best BA incision that will prevent me from losing sensation?
Sensation after breast augmentation
Aug 24, 2015
The incision is probably not the main reason for changes in sensation of the breast skin or nipple after breast augmentation, though peri-areola incisions have a higher chance of being associated with sensation changes than inframammary incisions. Changes are more related to the size of the implant and the amount of dissection needed to open the space for the implant. In many cases there are temporary changes in sensation due to the stretching of the skin and post-operative edema which…
When getting a breast lift, without implants added, just lifted, does that reduce the s
Volume after breast lift
Aug 24, 2015
These are good questions that should all be considered when you finally have a consultation with a plastic surgeon. In general, no breast tissue is removed when a lift is performed, simply the removal of a section of the skin so that the breast is not sagging as much, so the actual volume of the breast is the same. However, the distribution of the breast tissue is changed, with the tissue somewhat compressed against the chest, so it may appear at first to be slightly smaller….
Would I be a candidate for a breast augmentation?
Breast asymmetries
Apr 30, 2015
It is good that you recognize the asymmetry between the breasts before surgery so that you can discuss the various options for addressing it. There are several ways to deal with asymmetry including ignoring the difference and just augmenting the breasts, knowing that the addition of an implant will partially camouflage the difference, using slightly different sized implants to adjust for the volume difference, doing a small lift on the lower side with the more ptotic tissue and lower…
Facial sagging and caving in at 37. What is the best route for me to take?
Facial volume loss
Apr 30, 2015
I am sorry to hear you have been through so many recent problems and I hope your life is settling down a little. Facial volume loss is one of the most common reasons for aging changes in the face and it seems that you have two underlying reasons for the loss and changes you see in your face–loss of dental support to help support the cheek internally and weight loss with subsequent thinning of the subcutaneous fat and loss of fat in the fat compartments which can lead to some of the…
If I have more stretch marks on the upper area and the sides, will it be better to get
Stretch marks and tummy tucks
Apr 23, 2015
An in person evaluation would be the best way to answer your questions as it would allow evaluation of the stretchiness of the skin and the amount of diastasis recti you have, but in general, it is better to keep the incision low and pull the unmarked, upper skin down as far as possible. You will not be able to get rid of all the stretch marks no matter where the incision is, and the transverse incision still leaves a scar. In general, the rectus muscles are tightened during the…

Tummy tuck
Apr 23, 2015
In general a tuck will take about 2-3 hours, but the length of time needed for a procedure depends on the amount of tissue that needs to be removed and if there are ancillary procedures to be performed, so it is not really possible to fully answer your question. Recovery is a process and cannot be timed to the exact date. Again, depending on the amount of tissue to be removed and the amount of muscle tightening needed, time to driving may take 2-3 weeks and also depends on the…
Weiss/Anchor procedure vs. Vertical Incision or Lolliop?
Wise pattern reduction versus “lollipop”
Apr 23, 2015
Seeing a patient in person is always the best way to determine what style of reduction would be best as there are limitations to photos in evaluating the amount of ptosis and the quality of the skin. I do both patterns for reduction, sometimes finding that the vertical reduction needs a small lateral extension in the inframammary fold to fully lift the breast and in order to remove the amount of tissue needed to get you from a DDD to a C cup. I believe that this somewhat extended…

Facelift healing
Dec 18, 2014
I certainly hope you are keeping in touch with your surgeon during your healing after facelift. The internal healing time is always longer than one thinks, and there can be changes that occur over a long period and do not respond to treatment immediately. You can ask your surgeon what he thinks is causing the lines, as sometimes internal suturing can be related to what you see on the outside, or it could be related to the tightness of the skin or the tension on the skin which…
Do I need an uplift with implants, or are implants enough to fill out?
Breast lift with augmentation
Dec 18, 2014
From the picture you included it appears that your nipples are hanging below the inframammary fold, that is, you have significant breast ptosis. Without an “in-person” examination I cannot specifically recommend any treatment, but in general, you cannot get enough lift with just an augmentation and the breasts will appear to hang off of the implant and will not be a pleasing shape.
34 year old. Am I a candidate for lower blepharoplasty?
Eyelid bags
Dec 18, 2014
The photos you sent are somewhat limiting in diagnosing the full extent of the problem, as the projection of your cheek bones as well as the fullness, or lack of fullness in the mid-cheek can affect the lower eyelids and need to be considered in the options for treatment. You might benefit from surgery alone or perhaps a combination of therapies such as skin tightening and filler can offer improvement without the surgical downtime. There are different risks and benefits of each…
What is the best way to fill in my brow indentation?
Forehead contour problem
Nov 3, 2014
Without having an in person evaluation as to the cause of the indentation it is impossible to fully answer your question. There are filler materials that can provide either temporary or more permanent volume to the area, but the use of these different materials must be discussed in terms of all the risks and potential complications with your surgeon. It is possible that certain muscle function may be causing this as well, which could be improved temporarily with a neuromodulator….
Can I get a BBL, lipo, breast reduction and lift without the tummy tuck because I want
Combination surgeries-BBL, lipo, breast reduction/lift
Nov 3, 2014
It certainly is possible to combine the above surgeries without doing a tummy tuck at the same time, but whether this is the right combination for you can only be decided after a thorough consultation with your plastic surgeon. Waiting to do a tummy tuck until after completing child bearing is usually the best way to get the best results. You also might want to consider waiting on the breast surgery until after having your children, as pregnancy can also affect the size and shape…
