The pulling in the lower abdomen will improve over tome as the healing tissue remodels.  Wound remodeling is a normal part of the healing process and occurs in the direction of applied tension.  It may take up to 1-year for a wound to remodel.

A properly performed lower body lift restores shape to the gluteum by correcting ptosis (drooping).  This is achieved with “auto-augmentation” of the gluteum.  Simply pulling the skin tight during lower body lift will flatten the bottom as you have observed.

Arm lift, or brachioplasty, May be safely performed under local anesthesia or conscious sedation.  Usually, the tumescent solution used in the liposuction portion of the case provided adequate pain control to perform the skin removal as well.  Please be sure that your procedure is performed at an accredited surgical facility.

Following body contouring surgeries, compression is worn for 4-6 weeks.  If additional lengthy of compression is needed, then a second stage compression garment may be used.  Swelling may wax and wane depending on activity and diet.  If your incisions continue to be swollen, you may consider the advice of your Plastic Surgeon as the incisions may be developing into keloids.  If this is the case, silicone compression and steroids may help.

Provided you are in good health and cleared to undergo Anasthesia, then you should be able to safely undergo cspsulectomy and implant exchange.  You may consider transitioning the plane to a dual plane (under the muscle).

The bane of TT is addressing the fluids post-surgery and this is managed by the drains.  Immediate post-operative massages are not recommended as they may exacerbate fluid or seroma.  I do recommend Lymphedema massages for persistent swelling or soft tissue mobilization about 6-weeks after. This helps with any lumpy-bumpy feeling that is normal after any TT.  
Epidural anesthesia and foley catheter in tummy tuck. Is this good practice?

The reason you would have a Foley with almost any epidural is due to the possibility of urinary retention.This course of treatment strikes me as overkill.  Unless you are unable to safely undergo anesthesia ( breathing problems etc), GA is safe and does not significantly increase risk.  Our goal with TT is early ambulation (walking) after surgery in order to promote rapid recovery.  An epidural would potentially delay this as well.
I had an infusion in my back to my lower disc. Will that prevent a mommy makeover?

I am surmising you had an epidural or kyhoplasty for chronic back pain.  As long as you are able to walk after surgery and can be positioned properly for the procedure, your back treatments should not disqualify you from undergoing the MM.  I would recommend a thorough consultation with your Board Certified PS before considering any elective cosmetic procedure as well as clearing the procedure with your Pain Specialist/Radiologist/ Spine Surgeon.

Based on the photography, it appears the Right implant is displaced inferiorly causing derogation of the nipple.  This may be due to capsule on the right side or incorrect position from the original surgery.  I wonder if you may have had a hematoma on the right after the original augmentation.  I would suggest revision with capsulectomy and possibly alloderm to support the lower pole.  The implants do not need to be exchanged.  I would stick to smooth implants in cosmetic breast surgery (due...

The explant and breast lift may certainly be performed at the same time.  You have to be careful to discuss your goals with your Plastic Surgeon in detail in order that your expectations are properly understood.  In some cases, you may opt to consider fat grafting to the breast at the same time to restore some volume as removal of 700 cc is a drastic change in breast volume.
