You can have a septoplasty to correct your breathing. This is a very successful procedure if done correctly although the risk involved include infection, bleeding, septal perforation (1%), cosmetic change to the nose include nasal collapse/saddle nose deformity (1%) and numbness.
Correctly minor asymmetry can be quite challenging which is why fillers are sometimes the best way forward. Unfortunately all surgical procedures i.e. rhinoplasty/septorhinoplasty, carry a risk of asymmetry.
Going back to the same doctor for a nostril reduction after primary rhinoplasty?

Every surgeon will have his/her own policy on revision surgery and it is best to always inquire about this before undergoing primary surgery. Sometimes, having surgery can attenuate imperfections in another area, in your case the nostrils.
If this was not highlighted by yourself or the surgeon, one may argue it is a completely new procedure and a fee applicable. However, there is an argument that the width of your nostrils (or potential that it will look bigger after reduction surgery)…

I think your nose would be served best by undergoing a rhinoplasty. The nasal tip as you mention can be addressed by a combination of tip refinement and an alarplasty. However doing this alone with attenuate the dorsal hump. I would suggest having this augmented at the same time to give you a better profile and a softer more balanced nose.

With revision rhinoplasty, it is important to ensure you have set goals and your surgeon understands them and is able to achieve it, particularly as you have already had 4 procedures on your nose.
You do seem to have an under-resected dorsal septum than can be corrected to give you a better nasal profile. The columella overhang is likely due to an excessively long caudal septal edge or even a malpositioned columella strut which can be addressed. A layer of temporalis fascia will help…

Rhinoplasty is acknowledged as the most technically challenging cosmetic procedure. The are many good surgeons who perform this procedure regularly and that is the key – ensure that this procedure is performed regularly by your surgeon. It is expected that you will have the most swelling within the first 2 weeks after surgery. This gradually subsides over the following weeks. When to Stop Drinking Alcohol Prior to Surgery?

Every surgeon may have his/her own preference to this but I advice my patients to avoid any alcohol at least 48 hours before surgery mainly due to its dehydrating effect.
If you are heavy drinker, this is best discussed with your surgeon and certainly make him/her aware of this as this can affect you liver function and your bloods ability to clot, which is obviously important if you are having surgery.
