Breast  shaped implants are superior to round for the very reason you mentioned: natural shape! I always tell patients "if you are trying to make a breast bigger it makes sense to use a breast shaped thing to do it with. you wouldn't use a ball to make a box bigger!" The only con associated with shaped devices compared to round is that the surgeon MUST be experienced and the patient must be able to appreciate more natural appearance for slightly higher cost.

You absolutely have the right to privacy its called HIPPA rights. A letter from your plastic surgeon does  tend to give it away and if he does a good job your improved appearance will give you away. Get your plastic surgeon to answer the questions and then ask your primary care doc to submit from HIS office to maintain privacy. your doc may charges a fee for the service but its worth it...

In theory subglandular placement is best for the reasons you mentioned and because the female breast is located on top of the pectoral muscle. Your pics suggest that you have adequate breast tissue to accommodate a 375cc or smaller silicone implant in subglandular position.  Surgeon must evaluate your tissue thickness and dimensions of breast and implant to determine best fit. Your best bet is to go see your family Dr and ask his advice and treatment. Most local plastic surgeons are going to be reluctant to offer advice on a stranger's  work  (a Dr. They don't know) other than to go back and see your surgeon. If you had kept your business local the surgeons would likely know each other and collectively be more inclined to help you more directly.

I usually leave very small drains in  each of the places where I did the liposuction. I usually remove these drains about 4 days after surgery. How long it takes for drainage to disappear essentially depends upon how much fluid needs to drain out and how well it can drain out. If you are depending upon your body reabsorbing the fluid that becomes a situation your surgeon needs to watch and possibly help happen.

Fillers are expensive, temporary and unpredictable long term. Surgery is also expensive and will leave scars. Considering the very minor asymmetry you have my best suggestion is change your hairstyle and keep on living a happy life.

I think you and your Dr made a wise decision. It's 2 years later, you are fit and you have excess abdominal skin that needs to come off. You have had a remarkably good experience with your breast implants. Implants are artificial devices added to your body that at some point will require some sort of maintenance or adjustment. take it easy and decide what you want to do. Exchange or remove those implants.

I think you look great. Maybe you should simply relax and enjoy the improvement. Remember this advice: " the enemy of good is better" It would be a different story if you were in a lot of pain, or if the breast was hard as a rock.  You have implants in your breasts, not additional breast tissue. So enjoy  it.

How you heal from this point is largely unknown.  You will probably be able to feel the bony bumps and maybe even see them but the bone bumps usually remodel to a smaller size over time. You will definitely be less swollen. All you can do is gently squeeze /press the lumps with your fingers  to help with the remodeling periodically.... and be patient.

An implant (buttock) in a thin person always looks weird. The whole idea with buttock fat grafting is to augment the muscle to make it look more toned more athletic more muscular. it's very difficult to get symmetrical placement of butt implants, & complication rate is high. You should probably not do all those surgeries together. You need to be 5'6" or taller and fit. 360 lipo and tummy tuck not safe. " one shot" suggests you can only do this once and the result will have to be very very good if not perfect. Surgeries of this magnitude often have minor, sometimes major complications. Break it up into smaller pieces, that way if you need a minor correction it can be done later along with the additional surgery.
