Would you recommend buccal fat removal or Botox to the masseter?

Good morning,
Your specific case would not benefit from just either bucal fat pad removal or masseter Botox.
Evaluating you profile and frontal projection, I find that you have a weak chin projection and an asymmetric chin.
I would suggest to perform a sliding genioplasty for midline correction and profile enhancement.
Regarding your TMJ, we need to assess the diagnosis very accurately to determine if you have condylar hyperplasia. If that is the case you would need a specific treatment…

Is sliding genioplasty a good choice and am I a candidate?

Good day,
Genioplasty definitely is a very powerful procedure to enhance the lower third and define the middle third of the face.
Reviewing your photo I see that you have a class 2 profile. So after orthodontics without Orthognathic Surgery , sliding genioplasty is your only viable option.
I would perform a two step genioplasty to gain a good advancement and a good profile enhancement.
Please avoid placing chin implants since your case would not benefit from those.

Genioplasty is a very powerful procedure to enhance the lower third of your face. The dent that you are seeing is the consequence of the gap created by the osteotomy. That gap can be filled either with temporary fillers while bone grows and fills the site, or a custom made iimplant can be placed to make the contour of the chin accord to the mandibular bone segment.
In your case a revision genioplasty could also be performed very carefully trough 3D surgical planning.
Best regards,

The use of these devices for Orthognathic preparation is gaining popularity around the world . Nonetheless I firmly believe that you should have traditional ortho installed prior your surgery given that it’s easier for stability to be mantained trough the wire arch than splints.
There are many surgeons that don’t do that and have had success. But cases should be very well selected.
Talk to your surgeon about it beforehand.
Best wishes

Sadly removal of your wisdom teeth will not incur in any facial slimming effects whatsoever. Additional procedures might need to be performed for those results
Can my facial asymmetry (in the lower half of my face) be corrected without orthodontic surgery?

Definitely a case in which growth is still happening needs to evaluated very carefully. Many times you might have condylar hyperplasia and that can cause growth conditioning to define even more an asymmetrical face.
Orthognathic surgery is mandated on these cases and with a careful evaluation and planning, we can carefully accommodate your jaw bones and bite to a normal position

TMJ surgery can be done simultaneously with Orthognathic surgery if the case is needed.
To correct an uneven smile and facial asymmetrical proportions, only Orthognathic surgery is needed
