Recent literature suggests that the risk of clotting (DVT and/or PE) is higher in abdominoplasty patients than with other plastic surgery procedures, thus, your doctor may want you to be on blood thinners for a short period of time after your surgery. Some people make suture abscesses or "spit" sutures as their body is dissolving the sutures. Clipping or pulling out the offending suture can speed up the healing process. Massage can also help the sutures to dissolve and can positively influence the healing process. You should see your surgeon so these areas can be assessed and treated (with clipping for example).

While we have made strides in implant reconstruction in patients with radiation, there are many factors involved and the risks will always be higher than implant reconstruction in a patient that did not receive radiation. It is not always an option. You have had two failed attempts at implant reconstruction. This is not hard to imagine, then, that it would fail again for you. Your options now are no reconstruction or the addition of healthy non-radiated tissue for reconstruction.

Expanders are not meant to be permanent devices, but having said that, they can remain in place for several years if need be. However, the longer they remain in place, the higher the risk of rupture and deflation. If your expanders deflate, they may need to be replaced if you have significant contraction of the tissues before the implants can be placed. You have significant excess and stretched out skin. From your photo, it looks as though you need a full abdominoplasty or tummy tuck. This will give you a larger scar than a mini tummy tuck, but will allow more of your excess skin to be removed. You will also have a scar around your belly button.
