The exact cause(s) of capsular contracture (CC) are still not completely understood. In my experience (40 years) this would require at least a 2 staged reconstruction:

1. You have a very severe 'unitized" scar of your mid abdomen. In lay talk that means that the skin,subcutaneous tissues, and fascias and muscles are fused in one unit so they cause a severe indentation at rest or when you tense your abdomen or cough. This needs to be reconstructed with a multilayered revision.

You hit the nail on the head when you said " I want to be proportioned". Don't get hung up on the number of cc.s. Breast diameter is more critical in determining size and that will determine the volume of a properly PROPORTIONED BREAST–and slope (teardrop shape).

The short answer is NO. A consultation with a board certified Plastic Surgeon is definitely in order to discuss your thoughts. A thorough medical and family history with especial reference to breast problems as well as a physical exam are important components.

Of course pictures would be helpful, BUT only to address specific anatomical elements and how they might be addressed by surgery. The real answer always lies with the patient after an in depth consultation with a boarded specialist.

Maybe both! The archetecture of the bones and cartilages that make up the external appearance may also cause airway obstruction, Septoplasty can be combined with rhinoplasty, or they can be done separately.

The question about 1000 cc. breast implants is not could it be done ,but rather should it be done. Think about adding another 21/2 lbs to your chest and the ramifications on your spine , shoulder muscles, and skin before you do this-and theshape of your breasts as you age.

"Killing two birds with one stone" usually doesn't apply in this case because:

1. You already have scars from the breast surgery.

2. You most probally will need some other breast proceedure at that time,ie. mastopexy or capsulotomy.

3. Removal of prostheses will create "dead" space in the breast pocket which may communicate with the abdominoplasty disection causing problems.

I suggest doing both at the same time but through separate incisions.

A quick check list that I recommend:

1. Moisturize skin 2x daily with Calandula Lotion

2. Build up to 50 sit-ups per day to increase blood flow to rectii.

3. NO SMOKING OR SMOKE FILLED ENVIORMENTS!!

4.Ask your surgeon about taking calcium channel blockers

Of all the qualities you listed, factor out prestige because it is often manufactured by media. Qualifications like board certification in the ASPS or ASAPS indicate an important level of training and knowledge. However, in my 37 yrs.of experience the surgeon-patient relationship is also very important in obtaining a satisfactory result.

The surgeon and the patient must gel to form a" theraputic unity".

When a patient is considering breast augmentation basically she is unhappy with the balance, equilibrium and harmony of her breasts as they relate to her body. The reasons for this are extremely complex for any number of reasons. Only she knows this and thererfore her ideas must be central to any decision as to size.

So here comes the "but". It is the surgeons job to factor into her desires the surgical realities of the proceedure as they relate to her chest size body…

No matter how good the technique or meticulous the closure the surgical realities are stacked against a PERMANENT improvement. The removal of the outer spread scar will create a much larger circumferential defect. When this is pulled inward to join the smaller circumference of the areola something has got to give. either the areola spreads unestheticaly or the scar spreads as it did in your situation.
