Breast implants should be roughly centered on the nipple to provide a natural contour. If you do not have enough skin between your nipple and the breast crease then your surgeon should have lowered your crease to accommodate the implant. Even though you may have a high crease you may also have stretched out skin which may benefit from breast lift surgery. An experienced board certified plastic surgeon would be able to determine the best procedure to improve your concerns.
