Hello! Your breasts look beautiful! As time goes on, the weight of the breast implant will generally change the final shape of the augmented breast. Congratulations on losing 40 lbs!! If you lost the weight rapidly, your skin may not have had enough time to catch up. I generally recommend waiting 6 months to allow skin to contract. In my opinion, your breast skin has probably tightened all that it will tighten and having a breast augmentation alone may be all that you want. However, the breast augmentation may also enlarge the areola and accentuate the differences between the breasts. Lowering the fold under your left breast at the time of your augmentation may help the breasts look more symmetrical. Depending on how quickly and how long ago you lost the weight, your skin may still contract on its own. I tell my patients it generally takes six months for breast skin to contract and they may like how their breasts look if they wait. If you like how your breasts look in a bra but don't like how they look out of the bra then you probably like the size of the breasts.
