Your plastic surgeon should be able to submit for you with supporting documents of medical necessity. You can ask your PCP/GYN or other doctor to write a letter to include in that packet. It should describe the symptoms (like back pain and shoulder grooving) and what conservative therapies have failed (like chiropractic or medications). With the right documentation, it usually isn't too difficult to get covered.

I would definitely give it some more time (at least 6-8 mos) before considering more surgery. At only one month, your scar tissue will still be forming, causing the firmness and density. Good for you for considering breast reduction surgery. It will certainly help relieve your symptoms including back pain. As for what size you can get down to, it will depend somewhat on your breast shape/size pre-operatively. In your consultation with your board certified plastic surgeon, together you can decide what is an appropriate post-op size. You can do either surgery first. I'd consider doing the breast reduction first, because it will dramatically improve your quality of life. Your back pain will be reduced (if not gone) overnight! You'll definitely want to discuss this with your plastic surgeon, since doing both operations will increase the risk of nipple loss or numbness on that side.

These are all questions you'll have to ask your plastic surgeon, since the answers may vary. In general if you combine cosmetic with insurance procedures, the patient will be responsible for a hospital and anaesthesia fee as well as a surgeon's fee. Costs and discounts vary across the country. However, you will be saving in time off work and recovery time by doing them at the same time instead of separately.

I do tell all my patients, however, that if they're in the process of losing more weight they should lose all the weight possible before the surgery. That way, the surgery can give you the best results with the flattest, tightest tummy possible. There is an excellent chance that your insurance will cover this. Your plastic surgeon should take care of any preauthorization necessary. You may need to get letters of support for medical necessity from your other doctors. A lift is automatically part of the reduction procedure, so no need to worry about that!

Yes, they will almost certainly look a little different after surgery, but they'll be a lot closer to symmetric than they are now! With the underdevelopment on the left, there is none of the natural ptosis (sag) seen on the right. So you definitely need at least a lift on the right, maybe even a small reduction depending upon your size. Your plastic surgeon should be able to get the breast sizes and shapes much closer with the recommended procedures.
