Most peoples faces are asymmetrical. Thread lifting can be performed unilaterally in an attempt to create the appearance of facial symmetry.
The physician you consulted sounds sensible if they don’t want to “overfill” your face because it is ptotic (dropped and not overtly volume depleted). This sounds like a reasonable scenario for unilateral thread lifting.
