Really hard to tell much from this photo. If you have skin laxity and contour irregularities, you will almost certainly need an in-person exam to evaluate skin quality, laxity, tissue health, etc. before being able to provide options for revision surgery.
Can you advise on under muscle revision with new pocket?

Not sure about the question. But your breast implants are slightly bottomed out with the lower pole of the skin envelope being stretched over time. They do not look perfect, but they are symmetric and you do not have mastopexy scars. You can certainly change the position of the implant and elevate the implant on the chest wall and place them under the muscle. You would need a mastopexy at that point to allow the skin to drape around the implant properly.
Muscle spasms 11 years post-op – is this normal?

I would not say that this is normal. But it would be difficult to know if the muscle spasms are related to your implants or not without an exam or more information. If the implants are still soft, in normal position, and symmetric, then it is still hard to know what might be going on. If the implants have become hardened, asymmetric, or in a different anatomic position than before, then it would be more likely to be cause by the implants and a revision procedure may offer some relief.

Unless your frame is bigger than it appears in the photo, I would caution using such a large implant with a breast lift. Your breast skin has already proven that it does not do well with heavy volumes. I would asssume that the amount of skin laxity you currently have is due to having large breasts at some time in the past. It is really hard to tell what is going on with just the photos. It could be that you just have some scar tissue or a seroma pocket or a cyst from seroma fluid. If you did, those things may improve with time, massage, steroid injections, etc. But it could be residual weakness of the abdomen fascia or loss of integrity of the fascia plication if it was completed with the abdominoplasty. It is unlikely to be a hernia, but it could be. Really hard to tell without an in person exam or imaging.
