Everyone wants their breasts to look better, bigger, and younger. Sometimes you can only have 2 out of 3 at one time, and something has to give. I am sorry to tell you that this would have been much easier and cheaper to do BEFORE putting in your implants, and it is entirely predictable that your right nipple position would look worse with a high, round, full implant.

There is a long history now of surgeons trying to make the face look younger by lifting the deepest layer of the face at the level of the bone. There are many options for you, so the answer really depends on you. At your young age, unless you have had tremendous weight changes, I would generally expect the skin to spring back fairly well. Just removing the implant and leaving the capsule behind will likely give you the look of a very small implant as your body's natural fluid fills the remaining pocket. Explantation alone is very low risk, but removing the tissue capsule takes away some of the blood supply to the breast. That can modestly increase the risk of healing problems when done with a breast lift, but is still very low risk when done conservatively. Some lifts reduce the blood supply, and some don't, so your surgeon needs to pick the right one if you want a combined approach. I totally understand that sometimes we can't have everything that is planned, just because of physics and biology. But I agree with you that if a lift wasn't done, either you need a refund of that part, or you get it at a later stage.
