A cosmetic rhinoplasty evaluation requires photographs, as well as an examination of the inside of the nose, and a tactile assessment of the nasal cartilages and skin. If you do not snort drugs such as cocaine, the perforation was most likely associated to your accident 20 years ago. A hematoma or blood accumulation underneath the lining of the septum could have been the trigger for the perforation. Perforations do not necessarily need any treatment. You should be evaluated by an ear, nose and throat specialist.

One must first determine if you have adequate tissue coverage and then degree of ptosis or position of your nipples. The quality and amount of breast tissue will determine the optimal volume of the implants and the position of your nipples will tell what the best type of lift is.
