Statement: Dr. Fletcher grew up in the Pacific Northwest. He earned his undergraduate degree in Health Science at Walla Walla University and his M.D. degree at the University of Washington School of Medicine.

Dr. Fletcher then went on to complete a 7 year, integrated plastic surgery residency at the University of Pittsburgh, which consistently ranks among the top 1 or 2 plastic surgery programs in the country. The University of Pittsburgh is well know for training leaders in plastic surgery by way of its high volume and very broad experience. While at UPMC, Dr. Fletcher learned from some of the very best surgeon educators in the country. During his research year, he served as the Hand Transplant Fellow, taking part in the first bilateral female hand transplants performed in the United States. While at UPMC, Dr. Fletcher became obsessed with the mastery of surgical technique driven by the desire to offer his patients the very best care he possibly could. To that end, he sought out additional training by way of the following subspecialty fellowships:

Facial Plastic Surgery Fellowship. Dr. Fletcher became fascinated with facial cosmetic surgery while in residency. This interest drove him to seek out additional training with the late Dr. Mark Codner. Dr. Codner was a master of facial cosmetic surgery. His books, articles and lectures on eyelid surgery, facelifts, and rhinoplasty have been critical to the advancement of facial cosmetic surgery around the world. While under the tutelage of Dr. Codner, Dr. Fletcher learned advanced techniques in upper and lower eyelid surgery, face and neck lift techniques and rhinoplasty surgery.

The University of Pennsylvania Microsurgery Program. This training experience focused on advanced techniques in reconstructive microsurgery with an emphasis on breast reconstruction after breast cancer. Because it is among the busiest microsurgery programs in the world, fellows are able to partake in hundreds of complex operations, gaining vast experience in a short amount of time. Dr. Fletcher refined the skill of small blood vessel and nerve dissection necessary for perforator flaps like the DIEP, SIEA, SGAP, IGAP, and PAP. This skill enables the surgeon to harvest tissue for breast reconstruction while minimizing injury to the patient’s body. In addition, he learned how to shape this tissue into a beautiful, natural-appearing new breast. At Penn, Dr. Fletcher discovered his passion for helping women recover from breast cancer through reconstructive surgery. Because of this training experience, he is able to offer advanced and comprehensive care to his patients.

The Curtis National Hand Fellowship. This fellowship is known for its dedication to outstanding surgical education. At Curtis, Dr. Fletcher learned advanced techniques in fracture care, tendon reconstruction, arthroscopic surgery, microvascular surgery, nerve repair and transfer as well as reconstructive bone and soft tissue surgery.

Dr. Fletcher is committed to providing the highest level of surgical care to his patients. He believes that the patient-physician relationship is the most critical element of patient care and enjoys spending the time necessary to understand each patient’s concerns and surgical goals.

Answer Snippet
What type of breast lift would be most effective/ get the best results?
Areola reduction vs. donut lift; possible fat grafting
Aug 14, 2023
You are a candidate for an areola reduction or a donut lift. Which option is best suited for you depends on the position of your nipple relative to other landmarks.

There are limitations to how much I can reduce the diameter of the areola in one operation. An areola reduction involves making two circular incisions. The first incision follows the border of the areola. The second circular incision is centered around the nipple and sets the new diameter of the areola. If the difference…

Is it possible to have a second breast reduction?
Revision breast reduction
Aug 14, 2023
The short answer is probably yes. If you are not returning to the same surgeon, be sure to bring in your operative report to your new surgeon so that they will have some idea of which area of your breasts contains the dominant blood supply to your nipples. As always, an in person exam is necessary to determine surgical strategy that will work for you.
New pain in breasts 7 months post op
New breast pain months after breast augmentation
Aug 10, 2023
If you were my patient, I would want to know about these new symptoms and would ask you to come to the office for a detailed examination. More than likely, the pain is due to a benign process like perichondritis (inflammation of the covering of the ribs or muscle irritation). But, it is important to rule out other causes.
I have indentations, spider veins and hyperpigmentation after my augmentation. What can be done for
Scar revision
Aug 10, 2023
Based on your photos, I would recommend a scar revision, which can be performed under local anesthesia.
If I had a breast reduction with 400g removed & want old breast size back, can I get 400cc implants?
Breast augmentation after breast reduction
Aug 8, 2023
Breast implants do not compress the same way that human tissue does. Generally speaking, silicone gel breast implants provide more projection per unit volume than breast tissue does. That means a smaller implant than 425 mL will probably meet your goals.

Selecting the correct breast implant is critically important. For that reason, I ask all of my patients to bring in before and after photos (often sourced from realself.com) depicting patients with similar breast size/shape and frame…

I’m looking to go as large as I can, can I get implants without a lift?
Breast augmentation without a lift
Aug 8, 2023
Based on your photos, I think it may be possible to achieve a good result with implants alone. An in person examination will help you make the right decision for you.
9 month post op breast augmentation. My left breast looks like it has more upper pole fullness (Phot
Breast asymmetry after breast augmentation
Aug 8, 2023
Please post your before photos so that I can see the shape of your breasts prior to breast augmentation. Frequently, subtle differences in a person’s natural breast shape persists or is amplified after breast augmentation. Another common cause of a persistently elevated breast implant is capsular contracture. An in person examination will help diagnose the problem.
49kg, 5’1, could I be left with uneven bulges if I have a TT with shorter scar?
TT concerns
Jun 1, 2023
You are wise to be concerned about your contour with a short scar TT. I tell my patients that their scar length will be determined by what their body needs to achieve the best possible contour. I almost always prioritize optimal contour over scar length. Frequently, achieving optimal contour just means increasing scar length by 3-4 inches, which rarely causes problems if the scar is well positioned.

To optimize scar position, I have my patients wear the bikini bottom or panties they…

Should stomach be uneven, belly button be off to the side & hips 2 different shapes & sizes? (Photos
Belly button position and flank asymmetry
Jun 1, 2023
Most belly buttons are naturally positioned to the right or left of midline. I always show my patients which side of the abdomen their belly button resides on prior to surgery so that they know where it will be after surgery. In addition, most of the time the right and left side of the abdomen and the right and left flanks (hips) have different shapes before surgery as well. Again, I show my patients the differences using their own photos before surgery so that they are aware of their own…
Suture granuloma at muscle repair. Will this compromise the muscle repair?
Nodule after abdominoplasty
Jun 1, 2023
Your plastic surgeon would know better than anyone if their rectus plication technique would be at risk of failure if a suture were removed. Your plastic surgeon should, therefore, manage any complications related to the plication and/or your abdominoplasty.
7 months post OP & feeling pain & twinges in both my breast (right is more achy), can you advise?
Breast pain after augmentation
Jun 1, 2023
It would be helpful to know what position you implants are in (subglandular vs. submuscular), what type of implants you have (smooth round, textured anatomic etc.), implant size, and if you have experienced any complications during your recovery (hematoma, capsular contracture, muscle injury). Thank you for your question.
What reasons would fat be added just to the upper face? Can this still look natural?
Facial fat grafting
Jun 1, 2023
Facial fat grafting is a valuable tool for restoring facial volume and harmony in multiple locations. Please post standardized facial photos (front, oblique, lateral) to help me make an accurate recommendation.
Steroid injections for tummy tuck scar caused burst blood vessels & atrophy, what can I do?
Scar revision
Jun 1, 2023
You need a scar revision, which involves surgically excising the poor quality scar and performing a precise closure. The biological reason to expect a higher quality scar during a revision is that the skin has had time to stretch so the mechanical stress across the closure is lower and therefore has a lower probability of become hypertrophic.
Renuva vs Fat Transfer (Which is better:) ?
Renuva vs. Fat
Jun 1, 2023
I favor fat under most circumstances. Micro fat grafting techniques achieve 90-95% fat retention when performed by an experienced surgeon. How the fat is harvested, processed and injected is critical to achieving high retention. Chose your surgeon carefully.
Blue inner corners of the eyes – What is the best option to treat this?
Lower eyelids
Jun 1, 2023
Your photo suggests that you would benefit from a lower eyelid blepharoplasty, micro fat grafting, and eyelid skin resurfacing. However, standardized photos (front, oblique, lateral) with eyes in neutral position are necessary to give you a more detailed assessment.
