Based on your pictures, this does appear to be a retracted columella. You’re right, this can be fixed with the closed approach and is a much more minor procedure than the full open rhinoplasty. I would suggest that you’re evaluated first either in person or through a virtual consultation. This can be corrected at 6-9 months after surgery.

This is a common finding in people who have larger noses. The lower part of your nose (caudal septum, columella) are prominent, causing the crease. Another thing to answer is if you’ve have any nasal fillers (liquid rhino). I see this often in patient’s who had filler injected at the base of their nose. Best treatments are rhinoplasty and release of the depressor septi muscles. If you’ve had filler – I’d recommend melting it with hyaluronidase before the rhinoplasty.

The area you circled is referred to as the “soft tissue triangle.” It’s a very delicate area of the nose that can have some changes after rhinoplasty if it’s not supported during the surgery. Normally at 1 year out, you’ve seen a majority of the changes that may occur. You may go on to develop more changes in this area however. This is a reasonable thing to show your primary rhinoplasty surgeon and seek options.

Alar Base Reduction helps correct wide alar bases and alar flaring. It is normally done at the end of a rhinoplasty procedure. This is because all the maneuvers done during the rhinoplasty surgery can actually improve the nostril asymmetry and alar base asymmetry, and sometimes you don’t need alar base reduction. If you’re considering rhinoplasty – be sure to discuss all these points with your surgeon.

Nonsurgical rhinoplasty uses fillers to improve areas of the nose. Therefore the nose it typically larger. I don’t recommend nonsurgical rhinoplasty for that reason. Having said that, you have a nose that is anatomically great for surgical rhinoplasty to achieve a smaller / narrower / petite nose.

Mommy Makeovers are safe operations however they are associated with risks. The abdominoplasty (tummy tuck) portion is particularly the part with the highest risk.
