Statement: My experience allows me to deal with all types of rhinoplasty, but in the dialogue with the patient it is important to make it clear that there are more or less difficult cases and that a secondary rhinoplasty is much more difficult than a primary rhinoplasty . In the case of secondary rhinoplasty, in fact, or when I intervene to correct the result obtained with a primary rhinoplasty, the tissues have already been touched and altered and do not allow the same malleability of intact tissues. Therefore, the final result of a secondary rhinoplasty, while obtaining a substantial improvement, is never completely predictable and the patient must keep in mind that perfectionisms cannot be guaranteed.

I think that other pictures would be very useful to analyze completely the situation (profile and oblique views). In any case V inverted deformity seems to be present so revision rhinoplasty is indicated and spreader grafts and osteotomies are the two procedures which are able to get an excellent result.
Could I get good results from a tip plasty?

This nose shows the outcome of a previous trauma with crooked nose, dorsal hump and tip asymmetry. Maybe the trauma was the cause of a collapse of the two inferior third of the nose resulting in hanging columella. I think that the nasal septum is also deviated and septoplasty is indicated in order to straight the tip. Reshaping of the tip is not able to correct everything. In this case it seems so obvious to remove the hump and reshaping the tip but they are not sufficient to achieve an excellent result. In my opinion it needs cartilaginous grafts in order to reconstruct the framework of the nose and correct the outcomes of the previous traumas and if the septum is present, it is a very good source for the harvesting. Secondary rhinoplasty can improve many problems

The nose is too wide in the area of the middle vault, the spreader grafts have to be removed and sculptured  thinner and probably the osteotomies left some irregularities, so they have to be done again in lower position. The secondary rhinoplasty needs cartilaginous grafts harvested from auricular concha: they are useful to reshape the dorsum in a more natural way and hide irregularities. In my opinion open approach in your case can offer better guarantees of the result. You need a perfect reshaping of the tip with the right rotation: only  a stable complete view of the nasal structures can allow the precision of millimeters without asymmetries. I operated on patients with both approaches and the indication depends on the different clinical situations.
