Many thanks for your enquiry. Derotation of your nasal tip is certainly feasible through a revision rhinoplasty. These are never simple procedures and in your scenario, you are likely to need grafts to restore your nasal length and to derotate the nasal tip. This should help reduce the appearance of your nostril show but would be best performed by someone with significant experience in revision rhinoplasty.

Many thanks for your enquiry. From my perspective, I agree with your thoughts about your chin. As it is little set back, it gives the impression of your nose being too big. I suspect that your nose is not too far off reasonable projection but the dorsal prominence together with your chin being set back, give the impression on your nose being larger. To address things, I think that your would benefit from both being addressed to help harmonise your nose with your face.

Generally, a tip twist can be improved but the inherent forces arising in healing owing the memory of the underlying structure can pull things back to their pre-operative position. However, using grafts that have enough structural integrity together with addressing any associated septal deviation can aid in reducing the degree to retwisitng. It is possible for the tip to come down after surgery. However, your pictures suggest some supra tip fullness and with time, as this reduces, the general balance of your tip will seem improved.
