In general, hypertrophic scars are rare after Asian eyelid surgery. However, if you develop one, some of the treatment options include revision surgery, steroid injection, scar gel, laser( I personally don't recommend this) and observation until the healing is completed. Healing usually takes minimum of one year.

When you have a well defined fold on one side only, my recommendations are as follows: 1 If you like the side with the fold, then consider doing the double eyelid surgery on the other side only. 2 If you desire to change the size or the shape of the fold, then consider doing the procedure on both sides. 3 Use of tape may have a temporary benefit but I wouldn't recommend patients to use it for a long time as this may damage the skin.

I understand your concern but the truth is your crease height will definitely lower as swelling subsides over time. The question is how much will it lower. I understand it's not easy but you need toobserve for at least 3-6 months before starting to make an assessment on the outcome including the crease height.

Although this wouldn't be my first recommendation for someone like you, the fillers like Restylane or Juvederm may be injected along your hollowed lines of the lower eyelids to camouflage the bags. This usually last about 6-9 months in my experience. some of the side effects include visible lumps, bruising, redness, blue-tinged skin and rarely blindness.

Based on the picture, I would recommend transconjunctival lower blepharoplasty, release of arcus marginalis and fat redistribution. Done well, this gives precise, consistent and very satisfied results.

Treatment options include but not limited to filler injections, fat grafting, surgery and doing nothing. My recommendation is either transconjunctival lower blepharoplasty or no surgery. In my opinion, the benefit/risk ratio doesn't warrant fat grafting and filler injection.

In my opinion, if you a more predictable and longer lasting outcome, it would be transconjunctival lower eyelid surgery and fat redistribution. Filler injection and free fat grafting results are not as predictable as surgery.
